Quick answer: should you do the Fire Cape or the Inferno first?

Do the Fire Cape first — you literally have no choice. Entering the Inferno requires handing a fire cape to TzHaar-Ket-Keh as a one-time entry fee, so the Fight Caves is a hard prerequisite. What is the Fire Cape and how hard is it?

The fire cape is the reward for surviving the TzHaar Fight Cave, a solo minigame in Mor Ul Rek inside the Karamja volcano. You fight 63 waves of increasingly dangerous volcanic creatures — Tz-Kih (lvl 22), Tz-Kek, Tok-Xil (lvl 90), Yt-MejKot (lvl 180), Ket-Zek (lvl 360) — before the level-702 TzTok-Jad appears on the final wave (OSRS Wiki).

Jad is the whole test. He has three attack styles and hits hard — a max hit of 97 with his ranged and melee attacks and 95 with magic (OSRS Wiki) — but his attacks are heavily telegraphed. His magic attack rears back like a cobra; his ranged attack slams the ground. If you flick the correct overhead prayer (Protect from Magic or Protect from Missiles) before the hit lands, you take zero damage. Drop him to roughly half health and four Yt-HurKot healers spawn to top him back up — you have to pull them off and kill or trap them, or Jad heals to full.

Most players beat the fire cape with a Ranged setup (a blowpipe or crossbow), around 70+ Ranged and Defence, and Rune/Karil's-tier armour. It is genuinely learnable in an afternoon or two once your prayer-switching is clean. That's why it's treated as an early-game milestone rather than an endgame feat.

What is the Inferno and why is it so much harder?

You survive 69 waves of "Jal" monsters (the TzHaar word for "foreign"), including Jal-Nib nibblers that swarm you, Jal-MejRah rangers, Jal-Ak blobs that split, and the Jal-Zek magers. Then comes the level-1400 TzKal-Zuk, whose max hit is 148 and who fires a wide typeless attack you must pray against on a strict rotation (OSRS Wiki). During the fight a shielded Jal-MejJak set-piece and a final healer phase punish any prayer mistake. One wrong flick, one misclick, one greedy attack — and a run of 45+ minutes ends, sending you back to wave 1.

There is no checkpoint and no restocking. As the wiki notes, "if defeated, the player must restart from the first wave" (OSRS Wiki). That "back to wave 1" reset is why the Inferno takes most players dozens of hours across many attempts, and why the first cape is such a celebrated achievement. It demands near-max gear, deep prayer-flicking muscle memory, and the patience to eat repeated wave-67-plus deaths.

Is the Inferno worth it in OSRS?

For a maxed or near-maxed main doing high-level PvM, yes. The infernal cape is a permanent best-in-slot-tier melee upgrade, it's the flex every serious account wants, and once you've learned it you can farm Zuk as a Slayer task for solid XP (101,890 Slayer XP per kill). If you already have the gear and enjoy the prayer-flicking skill ceiling, it's the most rewarding solo grind in the game.

For a mid-level or returning account, no — not yet. The fire cape gives you the second-best melee cape for a fraction of the effort, and your gp and hours are better spent on the gear upgrades that actually raise your DPS first. Chasing the Inferno before your gear and mechanics are ready just means hundreds of failed attempts. Get the fire cape, gear up, and come back.
